Luteal Supplementation with Estradiol and Progesterone in Patients Stimulated with GnRH Antagonist/rFSH for IVF

چکیده
The use of progesterone for luteal support in stimulated cycles for in-vitro fertilization is well established. However, the benefits of the addition of estradiol are still controversial. The aim of this study was to compare ongoing pregnancy rates in patients stimulated with recombinant follicle stimulating hormone and gonadotrophin releasing hormone antagonist for in vitro fertilization, who received micronized progesterone for luteal phase supplementation, with or without the addition of estradiol . Thirty-five patients underwent ovarian stimulation with recombinant follicle stimulating hormone and gonadotrophin releasing hormone antagonist and received micronized progesterone for luteal phase supplementation. These patients were compared to 32 other patients who underwent controlled ovarian stimulation with recombinant follicle stimulating hormone and gonadotrophin releasing hormone antagonist and who also received micronized progesterone and estradiol for luteal phase supplementation. There was no significant difference between the two groups for mean-age, duration of infertility or base line follicle stimulating hormone, number of ampoules of gonadotrophin used, number of mature oocyte retrieved, estradiol concentration on the day of injection of human chronic gonadotrophin, fertilization rate or number of embryo transferred. Addition of estradiol to progesterone for luteal supplementation in patients stimulated with GnRH antagonist/rFSH for IVF: a randomized controlled trial.
BACKGROUND The role of progesterone for luteal support in stimulated cycles for IVF is well established. However, controversy still surrounds the benefit of additional supplementation with estradiol (E2) in GnRH agonist (GnRHa) cycles, while no such data are available for GnRH antagonists. The aim of this randomized controlled trial (RCT) was to compare ongoing pregnancy rates in patients stimu...

Background: This study aims to verify if luteal estradiol pre-treatment improves IVF/ICSI outcomes in a GnRH antagonist protocol as compared to a micro dose GnRH agonist protocol in poor-responding patients. Materials and Methods: A total of 116 IVF/ICSI cycles were included in this prospective randomized clinical trial. The selected women were randomly assigned to receive an estradiol pre-trea...
متن کاملLuteal estradiol supplementation in gonadotropin-releasing hormone antagonist cycles for infertile patients in vitro fertilization
OBJECTIVE To evaluate the effect of the addition of estradiol to luteal progesterone supplementation in GnRH antagonist cycles for infertile patients undergoing IVF/ICSI. METHODS One hundred and ten infertile patients, aged 28 to 39 years, were recruited for this prospective randomized study. They were randomly assigned to receive vaginal progesterone gel (Crinone) along with 4 mg estradiol v...
متن کاملAn update of luteal phase support in stimulated IVF cycles.
Stimulated IVF cycles are associated with luteal phase defect. In order to overcome this, different doses, durations and types of luteal phase support (LPS) have been evaluated. There is still no agreement regarding the optimal supplementation scheme. The aim of this paper is to assess the past and the current clinical practices of luteal supplementation in IVF. The databases of Medline and Pub...
متن کاملHighly purified hMG versus recombinant FSH in ovarian hyperstimulation with GnRH antagonists--a randomized study.
BACKGROUND Highly purified hMG (hp-hMG) has recently shown better cycle outcome than the recombinant FSH (rFSH) when compared in GnRH agonist long protocol cycles. However, they have not yet been compared in GnRH antagonist cycles. METHODS A RCT comparing the ongoing pregnancy rate (primary end-point) in 280 patients undergoing IVF/ICSI after stimulation with hp-hMG or rFSH controlled with a ...
